We are seeking an experienced, passionate, and highly motivated NDIS Support Coordinator to join our growing team. This is an exciting opportunity to work alongside participants, families, carers, providers, and community stakeholders to help individuals maximise the benefits of their NDIS plans and achieve meaningful outcomes.

As a NDIS Support Coordinator, you will work closely with NDIS participants to build their capacity, strengthen their support networks, and connect them with appropriate services and community supports.
You will support participants to understand and implement their NDIS plans while promoting choice, control, independence, and informed decision-making.
This role requires excellent communication skills, strong organisational abilities, and a thorough understanding of the NDIS framework and participant-centred practice.
Assist participants to understand and implement their NDIS plans.
Support participants to identify and pursue their goals and aspirations.
Connect participants with suitable service providers and community supports.
Develop and maintain positive relationships with participants, families, carers, guardians, and stakeholders.
Promote participant choice, control, dignity, and independence.
Support participants through plan implementation, service transitions, and changes in circumstances.
Monitor service delivery and ensure supports meet participant needs and outcomes.
Assist participants to build their ability to coordinate and manage supports independently where possible.
Strengthen informal, mainstream, and community support networks.
Support participants to develop self-advocacy and decision-making skills.
Facilitate collaborative planning and goal-setting processes.
Advocate for participant rights, safety, and wellbeing.
Assist participants to address barriers to accessing services and supports.
Support resolution of service delivery concerns and provider issues.
Assist participants during NDIS plan reviews and reassessments.
Maintain accurate, timely, and professional case notes and participant records.
Ensure all documentation meets NDIS requirements and organisational standards.
Monitor participant budgets and support utilisation where required.
Prepare reports and supporting documentation for plan reviews.
Ensure services are delivered in accordance with NDIS legislation, Practice Standards, and organisational policies.
Qualifications in Community Services, Disability, Human Services, Social Work, Psychology, Allied Health, Case Management, or a related field; or significant industry experience.
Demonstrated experience in Support Coordination, Case Management, Disability Services, Community Services, Mental Health, or a similar role.
Strong understanding of the NDIS and participant-centred service delivery.
Excellent written and verbal communication skills.
Strong organisational, problem-solving, and time-management abilities.
Ability to work independently while contributing positively to a team environment.
Current Driver Licence and reliable vehicle.
Current National Police Check (or willingness to obtain).
Current NDIS Worker Screening Clearance (or willingness to obtain).
Current Working with Children Check (Blue Card) or willingness to obtain.
Previous experience as a NDIS Support Coordinator.
Knowledge of psychosocial disability, intellectual disability, autism, complex care, and high-support needs.
Understanding of NDIS legislation, Practice Standards, Quality Indicators, and Code of Conduct.
Experience supporting participants through plan reviews, reassessments, and service transitions.
Experience using client management systems and NDIS portals.
