About the Role

Carbal is seeking an Aboriginal Health Worker or Aboriginal Health Practitioner to join our Toowoomba Clinic team.

This role is integral to the delivery of culturally safe, person-centred primary health care and plays a key role in supporting clients to access services, engage with care, and achieve improved health outcomes.

Working alongside GPs, Nurses, Allied Health Professionals, and other support services, you will contribute to clinical care, health promotion, chronic disease management, care coordination, and community engagement activities.

We welcome applications from both experienced Aboriginal Health Workers and Health Practitioners, as well as those who have recently obtained their qualifications and are looking to build their career within a supportive primary health care environment. Ongoing mentoring, professional development, and support will be provided.

Key Responsibilities

Clinical Support

· Assist with health assessments, including 715 Health Checks
· Support chronic disease management, care planning, and follow-up activities
· Assist GPs and Nurses with clinical procedures and treatment room activities

Client Engagement & Cultural Support

· Build strong relationships with Aboriginal and Torres Strait Islander clients and community
· Support clients to access health services, attend appointments, and navigate the health system
· Promote culturally safe, respectful, and person-centred care

Health Promotion & Community Engagement

· Support health promotion, education, and preventative health initiatives
· Encourage healthy lifestyles and early intervention strategies
· Participate in community engagement activities and programs

Teamwork & Clinical Administration

· Work collaboratively within a multidisciplinary team
· Maintain accurate clinical records and documentation using clinical software systems
· Contribute to quality improvement, accreditation, and continuous improvement activities

What You'll Bring

Essential

· Identify as an Aboriginal and/or Torres Strait Islander person
· Minimum Certificate III in Aboriginal and Torres Strait Islander Primary Health Care (or nearing completion)
· Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Health Practitioner) or willingness to work towards
· Demonstrated experience delivering primary health care services, or a strong desire to build a career in Aboriginal and Torres Strait Islander primary health care
· Strong understanding of health issues impacting Aboriginal and Torres Strait Islander communities
· Demonstrated ability to work effectively with Aboriginal and Torres Strait Islander people and communities
· Strong communication and interpersonal skills
· Ability to work effectively within a multidisciplinary team
· Ability to manage workload and prioritise competing demands
· Current Open C Class Driver Licence
· Current CPR certification
· Ability to obtain a Blue Card and National Police Check

Desirable

· Experience in an Aboriginal Medical Service or community-controlled health organisation
· Experience supporting chronic disease management and health assessments
· Experience using Best Practice or similar clinical software
· Additional training in health promotion, preventative health, or community engagement
· Previous experience in the health, aged care, disability, or community services sector
